Overnight Team Travel Documents

Parents-Below are the standard (four) forms parents need to complete for all Nordic team trips. Once filled out email to bloomingtonnordic@gmail.com  

1. -Diver Release form, 2 or 3.-Indemnification form if older or younger than 18. (Use one), 4.-Trip Cancellation form,  5.-Trip Health & Permission form

          Note: Driver Release form cant be fully filled out until #drivers are known and athletes slotted to them just before leaving on the trip. 

Chaperones-There are a couple of important steps to complete in order for you to serve as a chaperone and/or driver. Please see below. 

1. Register as a volunteer with the school district here  https://apps.raptortech.com/Apply/OTA2Njplbi1VUw== - NOTE: please start this process immediately as a background check will be required if you don't already have one completed by the school district.

2. Complete a "Chaperone indemnification Form" and send to bloomingtonnordic@gmail.com
